Overview
Immune-mediated polyarthritis (IMPA) is an autoimmune condition that affects multiple joints in dogs and cats. In this disorder, the pet's immune system mistakenly attacks healthy joint tissues, leading to widespread inflammation, pain, and mobility issues. Unlike infectious arthritis caused by bacteria or other pathogens, IMPA is a sterile condition where no infectious agents are present in the affected joints.
The condition can develop suddenly or gradually, affecting pets of any age, breed, or gender. IMPA is characterized by inflammation in the synovial membrane - the tissue that lines the inside of joint capsules and produces lubricating fluid. When this membrane becomes inflamed, it produces excess fluid and inflammatory cells, causing joints to become swollen, painful, and stiff.
There are two main types of IMPA: Type I (idiopathic) where no underlying cause is identified, and Type II (reactive) which develops secondary to other conditions such as infections, cancers, or drug reactions. Diagnosis

Physical Examination: Veterinarians assess joint swelling, pain, range of motion, and overall mobility. Multiple joints are typically affected, distinguishing IMPA from single-joint injuries.
Joint Fluid Analysis: This is the most definitive diagnostic test. Fluid is extracted from affected joints using a needle and syringe. In IMPA, the fluid shows increased white blood cells (primarily neutrophils) without bacteria or other infectious agents.
Blood Tests: Complete blood count and chemistry panels may reveal elevated white blood cells, increased inflammatory markers, and other systemic changes. Specific tests for autoimmune markers may be performed.
X-rays: Radiographs help rule out other joint diseases and assess for bone changes. In early IMPA, X-rays may appear normal, but chronic cases may show joint space narrowing or bone changes.

While IMPA cannot be completely prevented due to its autoimmune nature, certain measures may reduce risk:

The prognosis for IMPA varies significantly depending on the type, severity, and response to treatment. Type I (idiopathic) IMPA generally has a better prognosis than Type II, which depends on successfully treating the underlying cause.
With appropriate treatment, many pets experience significant improvement within days to weeks of starting therapy. However, IMPA is typically a chronic condition requiring long-term management. Some pets achieve complete remission and can eventually discontinue medications, while others require lifelong treatment.
Factors affecting prognosis include early diagnosis and treatment, compliance with medication protocols, and the pet's overall health status. Pets that respond well to initial treatment often maintain good quality of life with proper management.

Seek immediate veterinary attention if your pet shows:
- Sudden onset of severe lameness affecting multiple limbs
